The coronavirus disease (Covid-19) first appeared in the Wuhan district of Hubei province of China in early December 2019. The first case was reported by China on January 7, 2020, and this aroused variable interest worldwide, with most countries initially ignoring the novel infection.
Fortunately, Indian health authorities sensed the danger, largely because the country has always been alert to new infections. The scientific think-tank at the Indian council of Medical Research (ICMR) became active immediately and the first laboratory confirmed case was identified at ICMR’s National Institute of Virology (NIV), Pune, sometime towards the end of January.
A look at the world Covid meter shows that there is striking variation in mortality rates across countries, ranging from 0.2% to 15% depending on age, the smoking habit and pre-existing co-morbidities. It may be too early to tell, but in general, countries in the Northern hemisphere have faced the maximum brunt, and those in the Southern hemisphere (and those located proximate to the Equator) have so far escaped high infection numbers.
Three factors seem to be playing a role in the observed lower numbers in India with almost zero occurrence of severe Covid-19 cases (until now). First, broad-based immunity in the population due to the extensive microbial load. The Indian population has been exposed to a vast variety of pathogens, including bacteria, parasites and viruses leading to the generation of broad specific memory T-cells in the system, ready to attack additional foreign invaders.
For example, the three main killers of Tuberculosis, HIV and Malaria have plagued India, Africa and several countries in the Southern hemisphere much more than the European and North American nations. In the context of CoV-2 coronavirus, the beneficial role of chloroquine and hydroxychloroquine has been much talked about and debated, while there has already been an extensive usage of this drug at the community level in India — this too may ultimately prove beneficial.

Second, epigenetic factors that include environment and food habits may also play a beneficial role for countries such as India; much literature is already available in Ayurveda and other Indian systems of medicine on the definitive beneficial effects of Indian spices in augmenting immunity.
Third, and most important, is the possible role of immune response genes in the Indian population. These genes are collectively referred to as comprising the human leucocyte antigen system or simply, the HLA genes. Their main biological function is to present invading foreign antigens to the immune systems, since T-cells, which act as the body’s soldiers come into play only when pathogens are presented to them in a more formal manner in association with HLA genes.
In other words, the pathogen must first attach to compounds created by HLA genes before T-Cells attack it. If no such compounds are produced by the body, then the T-Cells are ineffective. As a consequence of the microbial load, the Indian population possesses a high genetic diversity of HLA, much more extensive than Caucasian populations. Indeed, studies by the author at the All India Institute of Medical Sciences, New Delhi, over several decades revealed the presence of several novel HLA genes and their alleles in the Indian population, most of which do not occur in other ethnic groups. Such genetic diversity of HLA could affect viral fitness.
The question then is: Why should genetic variation in HLA genes play a role in the Covid-19 progression? One hint comes from earlier studies in related viral diseases: Certain genetic variants of the HLA system provide protection against such viruses, while others increase genetic susceptibility to them. Another source of indirect evidence comes from recent clinical Covid-19 studies which showed that rapid T-cell response appears to be crucial for recovery from Covid-19, and reduced functional diversity of T cells in peripheral blood could predict progression of Covid-19.
The big question is: Does this give Indians a better chance at fighting the virus effectively? From the epidemiological data so far, it seems so (although much more extensive research is required). However, it is important for us to keep viral loads in check and below the threshold levels. In this context, the complete lockdown announced by the government is highly timely and most desirable. It is imperative that the virus replication cycle gets disrupted as early as possible before it gains numbers that may become difficult for us to counter.
To this end, the images of crowds gathering in several places whether for panic buying or interstate movements are disturbing. They could jeopardise all efforts and mitigate whatever natural advantages we enjoy.
The State must act fast to enforce the lockdown, even forcibly if necessary. India may be the outlier in fighting the coronavirus infection and succeed in keeping the overall numbers lower than the rest of the world with minimal deaths.

The novel coronavirus mainly attacks the lungs. But doctors have been increasingly reporting cases of another battlefield raging within the body: the heart.
More than 1 in 5 patients develop heart damage as a result of COVID-19 in Wuhan, China, one small study published March 27 in the journal JAMA Cardiology suggested. While some of these patients have a history of heart conditions, others do not. So what’s going on?
Cardiologists say several scenarios could be unfolding: The heart may struggle to pump blood in the absence of enough oxygen; the virus may directly invade heart cells; or the body, in its attempt to eradicate the virus, may mobilize a storm of immune cells that attack the heart.

„We know that this is not the only virus that affects the heart,“ said Dr. Mohammad Madjid, an assistant professor at McGovern Medical School at The University of Texas Health Science Center at Houston (UTHealth). The risk of developing heart attacks, for example, is thought to increase about sixfold when a person is infected with the flu virus, according to a study published in 2018 in the New England Journal of Medicine.
What’s more, during most influenza epidemics, more patients die from heart complications than from pneumonia, according to a review published March 27 in the journal JAMA Cardiology. Viral infections can disrupt blood flow to the heart, cause irregular heartbeats and heart failure, according to the review.

The most dangerous moment is in the end-phase of the influenza/viral infection when you cough out the bacterial remains from the bronchia, that´s when arrhythmias occurs.
So while it doesn’t „come as a surprise,“ that novel coronavirus called SARS-CoV-2 can lead to heart damage, it may be occurring more frequently in these patients than it does in people infected with other viruses, Madjid, the lead author of the review, told Live Science.
The double-edged sword
The virus might be directly attacking the heart.
„We’re seeing cases of people who don’t have an underlying heart disease,“ who are getting heart damage, said Dr. Erin Michos, the associate director of preventive cardiology at Johns Hopkins School of Medicine. Heart damage isn’t typical in mild cases of COVID-19, and tends to occur more often in patients who have severe symptoms and are hospitalized, she said.
Though the virus predominantly affects the lungs, it is circulating in the bloodstream; that means the virus could directly invade and attack other organs, including the heart, Michos told Live Science.

Both heart cells and lung cells are covered with surface proteins known as angiotensin-converting enzyme 2 (ACE2) — these molecules serve as „doorways“ for the virus to enter cells. But this enzyme is a „double-edged sword,“ she said. On one hand, the ACE2 molecule acts as a gateway for the virus to enter the cell and replicate, but on the other hand, it normally serves a „protective“ function, Michos said.
When tissues in the body are damaged — either by an invading virus such as SARS-CoV-2 or by other means, the body’s natural healing response involves releasing inflammatory molecules, such as small proteins called cytokines, into the bloodstream. But paradoxically, too much inflammation can actually make things worse. The ACE2 enzyme acts as an anti-inflammatory, keeping immune cells from inflicting more damage on the body’s own cells.
But when the virus latches onto ACE2 proteins, these proteins get knocked out of commission, possibly reducing the anti-inflammatory protection that they give. So the virus may be acting as a double-whammy by damaging cells directly and preventing the body from protecting tissues from inflammatory damage.
„If the heart muscle is inflamed and damaged by the virus, the heart can’t function,“ she said.
The novel coronavirus might also indirectly damage the heart. In this scenario, the patient’s immune system winds up „going haywire,“ Michos said. This scenario has played out in some really sick patients who have highly elevated inflammatory markers — or proteins that signal high levels of inflammation in the body.
This is called a „cytokine storm,“ Michos said. Cytokine storms damage organs throughout the body, including the heart and liver, she added. It’s not clear why some people have such an elevated response compared with others, but some people could be genetically prone to it, she added.
And then you have patients who have underlying heart disease who are at higher risk of developing severe symptoms of COVID-19 — and higher risk of mortality. „You can imagine, if their heart already has difficulty working … they don’t have the capacity to meet this challenge“ of not having enough oxygen because their lungs aren’t working as well.
So COVID-19 can „exacerbate“ underlying heart disease, Michos said. A new study, published April 3 in the journal Circulation, described four cases of heart damage among COVID-19 patients in New York, some with underlying conditions. (Michos is on the editorial board for the journal Circulation.)
Treatments and complications
Cardiologists identify heart damage using a blood test for a protein called troponin. When heart cells are injured, they leak troponin into the bloodstream. But „it’s sometimes not that easy,“ to figure out what kind of heart damage a patient is having, Michos said.
„We are really seeing different cardiac involvement,“ Michos said. So it matters „what’s causing the heart damage because you would treat it differently.“
For example, if the virus is directly invading the heart, the patient may need antiviral medications. If instead the immune system is causing heart damage, the patient might need immunosuppressants. Right now, no direct treatments target COVID-19, and most of the treatment being used currently involves supportive care such as providing more oxygen.
What’s more, people who have high blood pressure or other underlying heart conditions commonly take ACE inhibitors or angiotensin receptor blockers (ARBs) — medications that widen blood vessels, therefore increasing the amount of blood the heart pumps and lowering blood pressure.
Cardiologists are hotly debating whether people should stop or start taking those medications if they’re at high risk for COVID-19. (One paper suggested the drugs could be harmful, while some clinical trials are assessing the use of ARBs to reduce the severity of COVID-19, Live Science previously reported.)
It’s really hard to tease out whether having more ACE2 is helpful or harmful, as these proteins are how the virus enters the cells, but also known to protect the cells against injury, Michos said.
The current consensus is that if patients are already taking these medications, they should stay on them, she said. „Patients taking ACE-[inhibitors] and ARBs who contract COVID-19 should continue treatment, unless otherwise advised by their physician,“ according to a statement from the American Heart Association, the Heart Failure Society of America and the American College of Cardiology.
Experts from Australia and New Zealand similarly said they strongly recommend patients with hypertension, heart failure and cardiovascular disease who are already on these medications keep using them, according to a study preprint published on April 3 in The Medical Journal of Australia.
Complicating matters, certain drugs that are currently under investigation for treating COVID-19, including hydroxychloroquine — the drug that President Trump has said is a game-changer — could cause heart damage, those experts said. Now, the goal is to figure out if there’s a genetic or biochemical reason some people are more prone to heart damage from COVID-19 — and to figure out what drugs work best „to protect the heart from injury,“ Michos said.

Republicans have made tremendous political headway with their base through the term „fake news“.
It’s undermined confidence in the mainstream media and created an us-and-them perception of the relationship between the White House press corps and U.S. president Donald Trump.
But now, some Republicans are starting to use the term „fake science“ to describe what they see as a globalist attempt to shut down the economy and benefit Big Pharma and the government of China.
The most articulate advocate of this point of view is Shiva Ayyadurai, a Mumbai-born and U.S.-raised entrepreneur with a PhD in biological engineering from the Massachusetts Institute of Technology.
His academic research was focused on modelling whole cells. He was also a Fulbright scholar who studied how to integrate traditional medicine in South India with modern biological systems.
Ayyadurai is seeking the Republican nomination to run for the U.S. Senate in Massachusetts in 2020.
In a video posted on Twitter this weekend, he claimed that the United States has been „at war, politically“ as a result of the novel coronavirus.
„As an MIT PhD in biological engineering, it’s my view that the fear-mongering is really being used to suppress dissent, it’s being used to support mandated medicine, and it’s being really used to support crashing this economy,“ Ayyadurai alleged.

Ayyadurai has accused Fauci of representing the interests of a wide range of organizations, including the U.S. Centers for Disease Control, the Bill and Melinda Gates Foundation, the Chan Zuckerberg Initiative, the Clinton Foundation, the World Health Organization, and the government of China.
According to him, they’re all working in concert with Big Pharma, which makes money in three ways: through biomedical devices including diagnostic testing, vaccines, and drugs.
Ayyadurai said in his recent video that drugs aren’t paying as great returns nowadays because of lawsuits and the cost of obtaining government approvals.
He suggested that this has led the pharmaceutical giants to focus more attention on the two other areas: testing and vaccine development.
„This manufactured fear-mongered crisis—when you actually look at the numbers who’ve died, about 6,000, 7,000, 10,000, whatever it is—that number is so small compared to the 600,000 people who die from heart disease, the nearly 580,000 people who die from cancer, the 440,000 people who die from medical errors or the other 200,000 people who die from prescription drug issues,“ Ayyadurai said. „We’re taking an issue which has been completely hyped by Fauci to really drive an economic policy that’s to the detriment of the American worker.“
(According to the worldometer, there have been 68,419 deaths worldwide due to the novel coronavirus, as of this writing, including 9,444 in the United States.)
In another video on YouTube, Ayyadurai said that the World Health Organization, which created a diagnostic code for COVID-19, generates royalty revenues from companies that conduct diagnostic tests.
In contrast to government epidemiologists‘ advice to everyone to practise social distancing and stay at home to block the spread of the novel coronavirus, Ayyadurai argues in favour of „precision and personal“ medicine.
In an open letter to Trump last last month, Ayyadurai claimed that there is „no need to shutdown our entire country. There is no need to quarantine all of our citizens.“
Instead, Ayyadurai called on Trump to categorize citizens into four groups:
* those who test positive for COVID-19, who should be quarantined and given 400,000 IU of vitamin A palmitate per day for two days and 50,000 IU of vitamin D per day for two days;
* those who are hospitalized and in critical condition, who should receive the same treatments, as well as a 100-gram drip of vitamin C per day;
* those who are immuno-compromised. He thinks children in this group should receive the same levels of vitamin A palmitate and vitamin D as mentioned above, as well as 500 milligrams of vitamin C per day and three drops per drink of iodine/iodide once per day. Adults who are immuno-compromised, in his view, should receive double the dose of vitamin C and iodine/iodide as children;
* and those not in the above three groups, with children receiving 1,000 IU of vitamin A palmitate per day and 2,000 IU of vitamin D per day, along with 250 milligrams of vitamin C and three drops per drink, once a day, of iodine/iodide. He advises that adults in this group consume 10,000 IU of vitamin A palmitate and 5,000 IU of vitamin D per day, along with 1,000 milligrams of vitamin C per day and six drops of iodine/iodide per day.
„Healthy individuals based on the above protocol should get back to work immediately, and follow their daily program as aforementioned,“ Ayyadurai wrote to Trump.
This is the type of advice that is going to drive mainstream doctors and public-health professors into conniptions. But judging by Ayyadurai’s social-media following, it is gaining a foothold among libertarian-minded Americans who subscribe to his viewpoint.
In one YouTube interview, Ayyadurai even rejected the widespread view that HIV causes AIDS, also calling this „fake science“.
In the same interview, Ayyadurai expressed admiration for molecular and cell biologist Peter Duesberg, who was ostracized by other scientists after publishing a paper in 1987 questioning whether HIV caused AIDS.
Advocates for combination therapy involving antiretroviral drugs, such as Vancouver’s Dr. Julio Montaner, have published papers in peer-reviewed journals documenting how these treatments have turned what used to be a terminal condition into a chronic and manageable disease.
Ayyadurai, on the other hand, advocates a „systems approach“, which focuses on the immune system.
He describes this system as „complex“. To support this, he cited the existence of the innate immune and interferon systems, the adaptive immune system, the microbiome that includes gut bacteria, the virome of nearly 380 trillion viruses, and the gut-brain axis.
According to him, when people have a compromised immune system, the body’s defences overreact and attack its own cells. He has pointed out that this can occur after the immune system detects parts of an invader, like the novel coronavirus, left over somewhere in the body. He called this overreaction a „cytokine storm“.
Those with strong immune systems, on the other hand, can manage this, he insisted.
Ayyadurai accused Fauci of not providing the president with sufficient insights into how the immune system operates, leading to public policies that are wrecking the U.S. economy.
And as this is occurring, he claimed, China will use its vast amounts of U.S. currency to buy up undervalued American assets, including real estate.
„This will serve one country: China,“ Ayyadurai insisted. „China is a country that will profit from this.“
As of this writing, Trump has not fired Fauci.

We are in the midst of the strangest event of our lives. Societies have shut down. Families and whole nations face financial ruin. Walking the streets is now a crime from Paris to Sydney to Mumbai. And all of this has occurred not despite the will of the people but because of it.
The reasons are well known. There is a virus on the loose. It is transmitted by humans and is killing tens of thousands. It is an existential threat at which all resources must be thrown and all energy expended.

That data is provided by Italy, an early epicentre of the virus with many deaths recorded.
On March 26, the country’s peak health organisation — the National Institute of Health — published a report with details of the 6801 deaths the country had recorded to that point. This is a considerable sample size, and the figures are revealing.
The first statistics of note are those about the average age of casualties, which is 78. The median is 79. A little more than 95 per cent of victims were over 60…
Then there is the method of designating the virus as the cause of death, which includes anyone who had tested positive for it before dying. In other words, many were said to have died from the virus when in truth they merely died with it.
Third, 98 per cent of casualties of a random sample of patients had a pre-existing chronic illness, or comorbidity, at their time of death. About 21 per cent suffered from a single comorbidity, 26 per cent from two, 51 per cent from three and just 2 per cent with none.
Walter Ricciardi, scientific adviser to Italy’s Health Minister, recently reported: “On re-evaluation by the National Institute of Health, only 12 per cent of death certificates have shown a direct causality from coronavirus.”
The overwhelming majority of Italy’s deaths involved chronically ill and elderly patients.
This is not to diminish these tragedies. But the questions arise: why are we surrendering our hard-won civil liberties and committing economic suicide when this virus poses a danger to only a small portion of our society? Why do we not pour all of our resources into protecting the vulnerable?
The answer is that a 24-hour news cycle, with its morbid tallying of deaths, images of corpses and sensationalist reporting of outlier cases has whipped the public into a frenzy that politicians have had to take extreme measures to appease.
And anyone who questions the collective unreason is denounced on social media as a bloodthirsty mercenary who favours the economy over human life.
History shows time and again the reaction to a perceived crisis becomes the true catastrophe. Like the execution of witches until the mid-18th century or the scapegoating of Jews for poisoning wells during the Black Plague, evidence and logic are of no use to us now. There is an existential threat, and anyone who denies it is not just a denier but the cause.
None of this is to say this virus is not dangerous. It is. But the level of threat it poses is being exaggerated, and the response to it exaggerated as a result.
This is especially true in Australia, where infection rates appear to be relatively low and the government containment methods are among the most draconian worldwide.
If the government has compelling data to support this strategy, it should release it. But there seems to be no correlation between the scale of the threat and the economic and social damage we will suffer responding to it.
There is a disaster afoot. But it is not the COVID-19 virus. It is the putative remedy, a fact we will not appreciate until it’s too late.
